Blighted mushroom content expands with new gear, hazards, and rewards

Adds the level 60 Blightshroom Cap and Knight Mushroom Shield with the Shroom Bash perk, introduces poison-cloud combat behavior and effects for Blight Shroom, Blight Elder Shroom, and Blight Rootbeard encounters, refreshes mushroom-region loot tables and Boss Rush rewards with new drops, and reworks the former Mushy Visits quest into a level 40 Tall Cap Gorge expedition with updated dialogue and gating.
